It has been successfully used in many GP practices across the country.
We will have our group clinics for our patients within the primary care network, once a month starting at the end of April.
We have decided to first organise the group clinics for patients with Type 2 diabetes whose annual review is due. We hope, after a successful start, we will be able to offer group clinics to patients with other conditions (e.g. menopause, high blood pressure, HRT etc).
During the group clinics, there will always be a clinician (most times a specialised nurse) present who’ll go through the patients’ results individually and will be answering patients’ questions.
Data protection is paramount, and patients will be asked to sign confidential forms prior to the clinics to comply with the GDPR. Everything that will be discussed during the group clinics must stay between those taking part.
The sessions will provide opportunity for patients to meet other patients with the same health challenges, share experiences and advice on how to manage their conditions.
The clinics initially will be run online using MS Teams, prior to the clinics, all logins and technical requirements will be communicated to patients through a dedicated group clinics e-mail address.
Personal contact details will not be shared with participants.
Below are some videos explaining what group clinics are, how they run and the positive changes the clinics are making in people’s life.

Social Prescribers
Your social prescribing link worker can talk to you about a problem you have or a change you would like to make. If you’re feeling stressed by work, money, housing or loneliness, or if you’d like to make some lifestyle changes to help you manage a health condition, ask your GP or a member of the practice team to refer you for social prescribing. Your link worker will give advice, information and encouragement and connect you with suitable local services to support you.
Health Visitors for the Elderly
Health Visitors also care for the needs of Elderly people and are always willing to attend an elderly person by appointment to discuss any special needs that they may have.
Health Visitors
Health visitors are nurses who have had additional training to care for the family. Traditionally they have looked after the needs of mothers from 10 days after the birth, a point at which the Midwife relinquishes care. They are also the first port of call if you have any concerns about your child. Again, traditionally, they will remain involved in the care of your child until he/she is 5 years old.
Minor surgery
Appointments for this clinics can only be made at the request of the doctor or nurse. You will usually be asked to report to the receptionist and advice her that you have been asked to make the relevant appointment. An appointment date and time will then be sent to you. If the given appointment is inconvenient for you then please phone the surgery and rearrange it.
